Cancellation Policy:
We kindly request that appointments are cancelled with at least 24 hours notice. Missed appointments and cancellations with less than 24 hours notice will be subject to a fee of $50. Your insurance company will not cover this charge.
Billing Policy:
Your copay and any amount owed for an outstanding deductible will be collected at the time of the appointment. We ask that a credit/debit card is placed on file. We also accept cash or check. Payment may be made via phone or the client portal.
